The Social Security Administration (www.ssa.gov or call 1-800-772-1213) is the Federal agency responsible for coordinating the low-income subsidy, Extra Help. For people with very low incomes, Extra Help will help pay for premiums and will reduce your co-pays in Medicare Part D. You can even apply for Extra Help online.
